Care
/ Share Nutrition
Volunteers from the community assemble
and deliver food baskets each month to people in need.
Cosponsored by Lake Grove Presbyterian Church. Center
Client Service Coordinator Dana Tassos refers people
to this program.
Family
Consultations
Dana Tassos, the Client Services Coordinator,
confers with family members about issues such as health
and well being, safety, caregiving, housing, long-term
care, and respite services.
Grocery
Shopping Programs
The Center has two programs:
- For
housebound: Co-sponsored
with the Clackamas County Senior Citizens Council. Participants
telephone their grocery order to the Center on Tuesday
mornings between 10:00 a.m. and 12 noon, and on Wednesdays
volunteers shop, deliver, and assist in putting the groceries
away. A service fee of $3.00 per delivery is charged by
the Center.
- For
those who no longer drive: On
Monday afternoons the Lunch & Shop Program takes participants
by van to Safeway for shopping. Participants come in on
the morning van, have lunch at the Center, leave at 1:00
p.m. to shop for an hour, and return home. Contact Berta Derman at the

Delivered Meals
Whether meals are needed for just
a few days or on a long-term basis, Welcome Break volunteers
deliver hot meals every Monday, Wednesday, and Friday between
11:30 a.m. and 12:30 p.m. to older Lake Oswego residents.
Suggested donation is $4.00. Contact Dana Tassos.

Visits
Dana Tassos is available to help coordinate
services and assist families in resolving problems and
maximizing independence.
Information
and Referral
Dana Tassos, Client Services Coordinator,
provides information, explanations, and referrals to
community resources and services.
Long-Term
Care Options
The ACC coordinates with Clackamas County
Social Services Division, Area Agency on Aging, which
provides information, placement, and financial information
on a variety of living arrangements. Contact Dana.
Medicare
/ Health Insurance Help
A volunteer SHIBA (Senior Health Insurance Benefits Assistance)
consultant provides help with Medicare claims and appeals
by appointment only. The volunteer can also clarify terminology
and provisions of insurance policies owned or under consideration
for purchase. Call Berta Derman for an appointment.
PROBLEM
SOLVING
Dana Tassos
will meet with individuals to evaluate current and/or future
needs, and assist in resolving problems.
PROJECT ALERT
This is a registry cosponsored by the Lake
Oswego Police Department for anyone who wishes to receive
a daily telephone call to verify he or she is all right
during harsh weather or other emergency conditions in the
community. Contact Berta Derman.
PROTECTIVE
SERVICES
The Client Services Coordinator links individuals
to the proper authorities when they are no longer able to
manage their own affairs or when they may be the victim
of elder abuse (psychological, financial, or physical).

REFERRALS
Dana Tassos makes referrals to appropriate agencies on
clients' behalf to meet their current or future needs. Follow-up
assures that the requested services are being provided.
SENIOR
COMPANION PROGRAM
Older volunteers offer companionship, friendship,
respite services, and help to others. The Client Services
Coordinator, Dana Tassos, arranges for Senior Companion
visits.
