Filling out and submitting a Dog License Application

Step 1:

Fill out the dog license application form by either:

• Printing it and fill in by hand,  or
• Complete the form in Acrobat Reader.  (You may want to save a copy for future reference)

Step 2:

Determine type of payment:

• Mail in/hand deliver the application with a check, or
• Fill in the credit card information.

Step 3:

Submit the application via one of the 2 options.

• Mail to City Hall:

  Lake Oswego, OR  97034
  

       -or-

• Hand deliver it to City Hall during regular business hours.  Come to the Finance department on the first floor.
