Use the paper form under Supporting Documents below to complete Skate Park membership registration information and either drop it off at the Parks & Recreation office at 4101 Kruse Way, Lake Oswego, fax it to 503-697-6579 or drop off at the Skate Park.  You may also pay online using the link below, but the form still needs to be completed and turned in.
