Description:
Emergency Preparedness Meetings
The Bryant Neighborhood Association has prioritized Emergency Preparedness as one of our key
activities for the upcoming year based on a neighborhood survey conducted in 2018. We have
identified 40 individuals interested in learning more about how to be prepared in the event of an
emergency and how we should best respond to assist neighbors, if need be. We are now meeting
monthly to review information provided by PrepLO. These meetings provide a wealth of
information for residents on how to prepare for an emergency. Topics range from water
preparedness to basic first aid. We have two volunteers leading the effort to organize these
monthly meetings, Kassie Clarke and Ann Savage. We have identified free meeting space at the
LO Maintenance Center. Despite using multiple communication channels including email,
Facebook, and Nextdoor, we need to promote more awareness of our meetings and the
information being shared. To accomplish this, we seek grant funding to procure 10 neighborhood
reusable signs, a banner, and two flyers to be mailed to all 800 residences in the Bryant
Neighborhood. These mailings are in addition to the two mailings already provided by the city.
We also plan to promote our monthly meetings at a National Night Out Get Together in August
2019. This gathering will allow us to meet face-to-face to get to know one another, which is a
critical component prior to working together in the event of an emergency.
Emergency Preparedness Trainings
We have identified 10 neighborhood Team Captains, who are dedicated to Map Your
Neighborhood and be street leads in the event of an emergency. Over the course of the next year,
we plan to coordinate dedicated trainings on how to communicate and coordinate resources in
anticipation of a major disaster. Grant funding will be used to purchase 10 sets of two-way radios
to be tested quarterly with neighborhood Team Captains. We plan to follow the example of other
neighborhoods that have developed a communication plan for their neighborhoods to respond to

an emergency. Grant funds will also be used to secure basic supplies needed for our Team
Captains to be able to respond to an emergency. We will purchase two sets of Team Captain
Supply Kits Level 1 and one set of Level 2 Kit supplies as recommended by PrepLO, see
attached lists. We will purchase 1 of each item unless otherwise noted on the attached lists. One
modification was recommended by the review committee is to replace the LifeStraw Community
with the LifeStraw Mission Water Purification Systems. We can purchase four Mission Systems
for less than the price of one Community System and they can be distributed throughout the
neighborhood in the event of a disaster instead of kept at one location. All supplies will be kept
in a locked shed located at 16884 Bryant Road. All Team Captains will have the access code to
be able to access supplies. Quarterly drills will test accessing supplies by all Team Captains.
Team Captains are responsible for also identifying a backup lead that will also be involved in all
trainings. Team Captains will receive a two-way radio and also secure a GMRS license to
increase range of communication. We will enlist help from other neighborhood associations that
have already tested these methods to help our neighborhood design and implement the best
emergency response for the Bryant Neighborhood. The final radio model is still to be
determined; however, we will depend heavily on the experience of others and have received
strong support from other neighborhood associations currently using the model detailed in the
budget. We will use the expertise within PrepLO to train our neighbors on using and accessing
these supplies on a routine basis. The totals listed below are based on current estimates received.
We will strive to find the best pricing at the time of purchase to maximize the procurement of
supplies to properly equip our neighborhood to be able to respond to any disaster.

Section 3: Criteria
1. The project will provide a public benefit to the Lake Oswego community.
The Bryant Neighborhood Emergency Preparedness project adopted goals developed from a
neighboring neighborhood association that clearly outlines the public benefit to our
neighborhood:
1. Create an emergency response network on each street/area in our neighborhood and
through this network prepare to respond to any disaster such as a heavy winter storm or a
catastrophic earthquake (Cascadia Subduction Earthquake).
2. Prepare neighbors for individual and household survival in the event of an emergency or
disaster.
3. Bring neighbors together with a shared common goal of helping one another during and
after an emergency or catastrophic event.
2. The project has neighborhood/community support and committed volunteers for
implementation.
This project has support from 10 dedicated Team Captains, in addition to, two very dedicated
project coordinators, Kassie Clarke and Ann Savage. Both Kassie and Ann are CERT trained and
attend PrepLO meetings. They have successfully organized community meetings and are
dedicated to continuing this effort throughout the year. We also have 40 interested community
members.
3. Responsibility:
As stated above, Kassie Clarke and Ann Savage will lead all efforts related to emergency
preparedness for the Bryant Neighborhood Association.
4. For physical improvements, be located on public land.

Funds will be used for shared resources for all Team Captains and used quarterly for training
purposes.

8. Neighborhood parties, picnics and other social events are limited to $300 in grant funds.
We have limited funds to $300 for our National Night Out event on August 6, 2019. This event
will provide an opportunity to raise awareness about BNA, about what we’re doing, and most
importantly, will give neighbors a chance to meet and get to know each other, which is
significant when it comes to disaster prep and mapping the Bryant neighborhood area.
9. The project can be completed and expenses incurred by June 30, 2020, the end of the
grant cycle. All invoices and/or requests for reimbursement submitted to the City no later
than July 10, 2020. Provide the City with a project report by August 28, 2020. Include
photos if applicable, we like to brag!
TIMELINE: The project will be completed by June 30, 2020 with project report to the city
before August 28, 2020. As soon as grant funds are awarded, supplies will be purchased and
distributed accordingly. Neighborhood signs and banner will be printed so they can be used
immediately for neighborhood meetings.
Additional Considerations:
10. Describe any past projects that your neighborhood has completed through a
Neighborhood Enhancement Program grant.
We have a new Neighborhood Association board. To our knowledge, the Bryant Neighborhood
has not received significant grant funding in previous years. We have a new and engaged board
and are excited to implement this new project.
11. If applicable, describe matching funds or in-kind contributions.

PURPOSE:
The purpose of the Neighborhood Enhancement Program (NEP) is to support partnerships
between the City and neighborhoods by providing grants to qualified organizations for projects
that provide a public benefit to the community.
ELIGIBLE PROJECTS & FUNDING:
Grants will be awarded to projects meeting the criteria outlined on p. 1 of the NEP Program
Guide. Project ideas can be found on p. 2-4 of the Guide.
The City’s proposed funding for the Neighborhood Enhancement Program is projected at
$60,000 for the 2019-20 fiscal year. Grant awards will generally be limited to $10,000 per
neighborhood. If your association is partnering with another association on a larger project,
please note the partner organization/s under Requested Grant Amount and Neighborhood
Association below.

Description:
This request is made up of two separate projects which are described below.
FANFH Annual Meeting
The neighborhood association will be hosting our annual meeting in Fall. We found that
holding the annual meeting at this time of year substantially increased our attendance over
previous years when we held the annual meeting in Spring.
FANFH Emergency Preparedness Program Development
FANFH is initiating a neighborhood emergency preparedness program. FY2019-2020 efforts
are for program development, neighborhood involvement, project identification and
materials development. Deliverables will be an overall program plan, a neighborhood
Emergency Preparedness and Response Plan and various support materials. The
development and implementation of this plan is intended to be inclusive and open to all in
our Neighborhood.
Budget:
Here is a estimated budget for this request:
Overall total for both projects – $1842.00
Annual Meeting - $300.00
We are asking for $300 of grant money to provide catering and room costs for the annual meeting. Although
we typically try to secure the ACC, we are still assessed a cost for clean up. This would break out to $250 for
refreshments and $50 for clean up.
FANFH Emergency Preparedness Program development (see below and attached estimate from Office Depot.
This grant request is to fund reproduction costs for plans and other materials, postage, meeting and costs.

SECTION 3: CRITERIA
Please provide a narrative describing how your project meets the criteria listed below.
1. The project will provide a public benefit to the Lake Oswego community.
Annual Meeting:
We try to provide a benefit to the neighborhood during our meeting often by
arranging a guest speaker, in addition to holding officer elections and addressing
neighborhood topics. Last year we had a community resource panel of presenters.
The event was well attended, approx. 50 people, and well received. There was a lot

of discussion and questions of the panel. Each year we try to find a way to build
community and inform our residents on some relevant topics.
FANFH Emergency Preparedness Program development
This project is intended to improve the emergency preparedness of the residents in
FANFH which improves quality of life and provides neighborhood resilience. All

Hi David,
The Prep LO NEP grant review committee met today and discussed your application. No
problem on the $300 meeting expenses but we did hit a wall with the printing. I contacted Erik
Olsen and got this response from him: "Regarding the printing question, there is receptivity to the
idea to help fund printing services for outreach efforts related to emergency preparedness. As with other
proposals, the City Hall Print Shop could be utilized for printing services.”
I think your application states your NEP grants purpose and what you intend to work towards
as far as emergency preparedness and response planning. We are recommending that FAN/FH
stop by city hall and talk to the print shop (2nd floor) about your printing needs and the grant
request. They might request that you add something to the application? or that you need
someone’s blessing? You might want to get this in writing to add to the application when
submitting. This is a gray area but the printing costs don’t appear to be needed in the
application. Further follow up with Erik on this is recommended.
I’m not sure if Prep LO needs to review your application from here on out. Good luck from

I just spoke with Erik Olson at the City regarding the FANFH
request for an NEP grant this cycle to support our initial
neighborhood emergency plan development and other
planning during FY19-20. We are looking at reproduction,
postage and meeting costs during this period which we are
unable to fund from neighborhood sources. Eric advised
that, as part of the process, we need to meet with your team
to review our project idea and, essentially, obtain your
documented approval. We would then need to meet with
the City and obtain written approval from them to go into
our NEP application. Eric also advised that he did not believe
that NEP funds had been used for planning costs in the past
but he did not rule it out. PrepLO support to this concept
would be significant in getting this NEP grant approved.
So, when would we be able to meet and discuss my project?
I work from home and, thus, have a rather flexible schedule.
I just live across the street from the ACC. Please let me
know when and where were can get together so that I can
determine if this request is a go.

SECTION 2: PROJECT DESCRIPTION, BUDGET & LOCATION IF APPLICABLE
Description:
(Clearly describe your project in as much detail as possible. For neighborhood signs, please

FANFH is a neighborhood that has been experiencing extensive redevelopment over the past 10
years. With this redevelopment has come the removal of many of the large beautiful trees that
help identify our neighborhood.
Recently due to tree failure, the City of Lake Oswego Maintenance staff had to remove a very
large 150 year old elm heritage tree on First Street, to protect public safety and private
property. This was met with great sadness from the neighborhood and surrounding community
since many people were familiar with the massive beautiful tree that towered over the
buildings and provided a stunning canopy over the entire width of the street.
This is a two part project, with a vision to honor this beautiful tree, add value to the
neighborhood and create a place for community to gather.
Part l
Working with City staff large portions of the tree were saved in hopes of using the wood for
future projects. Also during tree removal, a section of the stump was left in place with the
intent of creating a bench out of the stump at that location. This would include brass markers
inset into the stump surface at various age ring locations noting significant events throughout
the history of the 150 year old tree. Events like the planting of the tree in 1869, 1910
incorporation of the City of LO, the Columbus Day Storm of 1962, and the first moon landing.
With the help of a committee, made up of NA members, Nicole Nathan, Executive Director for
the Arts Council of LO, and City staff the final design will be refined. One local wood artisan has
already agreed to donate his labor to create benches. Also as part of the elm stump project,
the sidewalk would be rebuilt to meet ADA compliance and reduce the dangerous trip hazard
that exists today.
Part ll
This would include one or two additional benches constructed using the elm wood for
placement at the Stafford Grove Park, located on City Park property behind Forest Hills
Elementary School in the Forest Hills area of the neighborhood. A basalt bench had previously
been approved at his location but has yet to be installed. These elm tree wood benches would
be built using boulders and slab wood which would complement the natural nature of this park
while also providing a relaxing place for visitors to sit.
With all the redevelopment in the neighborhood, the possibility of finding suitable boulders is
high, including the demo site next to City Hall scheduled to begin in June 2019. Photos are
included for both portions of the project to help better explain the vision.

Budget:

are purchasing more than $5,000 in goods or services from a single vendor/contractor, submit
copies of your bids with the application. See Program Guide p. 4, Step 7.)
We are asking for a grant amount of $9,955 to cover the cost for both portions of the project.
We have worked with Maintenance staff to develop project costs for sidewalk repair and tree
removal as well as the Arts Council who have provided the name of at least one artisan willing
to provide stump sculpting work at no cost.
Part I: Sidewalk Repair, Tree Removal and Art Piece
Based on two conversations with portable wood milling operations, and the recommendations
from Tree Care, (the city's tree contractor) we feel confident we will be able to have one or two
slabs created for additional benches at Stafford Grove Park.
$2,500 - 100' of sidewalk repair- (estimate from Rachel Sykes, Maintenance, email
included)
$3,000 - wood removal, stump grinding and landscape restoration with natives and low
maintenance plants and bulbs (estimate from Rachel Sykes, Maintenance, email
included). Some plantings may be provided by the abutting property owner who is
extremely supportive of project.
$50 - Brass markers
Part ll: Stafford Grove Park bench(s)
Based on project estimates from the 2018 grant application for one basalt bench with
landscaping, and resent conversations with city staff, we are confident the same budget would
be applicable for this portion of the bench project for 2019-20 grant cycle.
$3,500 - milling of elm wood bench and boulder placement
$905 - contingency

SECTION 3: CRITERIA
Please provide a narrative describing how your project meets the criteria listed below.
1. The project will provide a public benefit to the Lake Oswego community.
(Describe how your project will benefit the neighborhood and/or community. This is
your opportunity to demonstrate the benefit and value of your proposed project.)
As a whole, these two projects contribute to the health and vitality of our evolving First
Addition and Forest Hills neighborhoods by creating a sense of place. We want to
continue to preserve our past while embracing our ever evolving neighborhood.
Honoring this once noble elm tree in two unique ways fits how many of us view our
neighborhood and community while providing two wonderful positive gathering spaces.
And bonus, will be a small but mighty history lesson as well!
Funding the creation of one or two benches at Stafford Grove will complete this small
charming reflective park space, which was created in 2014 in honor Oregon Poet
Laureate, William Stafford, who once lived across the street. His little house did not
stand the crush of redevelopment and no longer remains, but this park and hopefully
the addition of a bench or two will retain an important piece of community identity.

The FANFH Neighborhood Association in the April 3, meeting voted to approve both
portions of the project and requested grant amount.
Nicole Nathan, Executive Director for the Arts Council of LO, has also provided support
for the project and agreed to assist with artisan referral and provide her public art
expertise to the project. An email from Nicole is included as part of the application.
City Maintenance staff, including Anthony Hooper, Public Works Director, Jim Bateman
and Rachel Sykes have all provided input, estimates and support for the project.

Working with Maintenance staff, Arts Council, FANFH, and volunteer artisan, the first
meeting is planned for late May to finalize design details. The project is anticipated to
begin in mid-summer with completion date no later than May 2020.

The FANFH NA has received grants consistently since the beginning of the program,
creating many beneficial projects and events.
One of the first projects awarded funds from the NEP, was the traffic roundabout at the
corner of 4th and C, adjacent to Rossman Park. While not perfect, it has created a
beautiful entry into the neighborhood and does provide traffic calming as designed.
With NEP funds the creation and installation of Stafford Grove Park poetry stones was
achieved, helping to create a valued public space used daily by students and members
of the community.
NEP funds allowed for the development of Candidates night at the Adult Community
Center. Candidates running for City Council positions were invited to meet members of
the entire community, not limited to FANFH neighborhood, and answer questions in a
sort of speed dating format. This was so successful other organizations now use this
model and FANFH hosts one of these events during every election cycle.
Thank you to the NEP Grants for providing the community a resource to explore ideas
and make the community a better place to live for all.

The FANFH Neighborhood Assoc. has discussed the proposal to have a bench
designed and constructed from the remaining trunk of the historic elm on
1st and B in First Addition. We also discussed the proposal to make two
simple benches from the salvaged wood taken from the historic elm with
masonry supports to be placed in Stafford Grove in Forest Hills. We voted
to support both of these proposals at our April 3rd coordinating committee
meeting.
We are a neighborhood that is passionate about walking. Realtors tout our
high walkability index. Benches allow walkers to rest and keep on walking,
especially in the hot summer sun. We are also a neighborhood that is
passionate about trees. Stafford Grove has been the site of recent Arbor
Day festivities. Repurposing an historic tree and highlighting its
continued presences in ITS neighborhood is a symbolic commitment to the
value we place on significant trees.
I hope the grant evaluation committee can find its way to supporting this
proposal.

DESCRIPTION:
The GNA would like to continue to build its long-standing successful ivy-pulling program. City funding
support in recent years has allowed us to improve participation, enlarge ivy-pulling areas and add Scotch
Broom to invasive species to be removed. From our meager start of approximately 10 devoted
volunteers, we were able in 2015 to count over 50 volunteers and post-pull BBQ attendees. In 2018, we
were able to bring out close to 40 volunteers for the effort, which was entirely volunteer driven this
year.
Our neighborhood is proud of our many mature trees and our extensive “almost” ivy-free stream
corridors, Willamette River front and treed common areas. Our area of focus remains Glenmorrie Drive
stream corridor, Old River Road river banks, Highway 43 common areas and Glenmorrie Terrace
greenway.
BUDGET:
Estimated Budget: $800
- Food and Beverage: $400, which helps provide water and snacks for volunteer workers as well as the
after-work gathering.
- Mailings and Fliers: $200. We would likely use one of our free mailings from the city, but would request
an additional one and some signage to help promote the event.
- Dumpsters: In the past, we have had up to three Dumpsters delivered for several days to help
accommodate yard debris pickup in the neighborhood, as well as overall neighborhood cleanup. These
may be available through other city programs.
- Garbage charge: $100, for any additional refuse pickup requirements as a result of neighborhood trash
picked up during the event.

Dear City of Lake Oswego,
On behalf of the board of the Glenmorrie Neighborhood Association, I would like to offer a letter of
support for the Neighborhood Enhancement Program grant being submitted by our association.
Since I have lived in the neighborhood, (2007), I have participated in almost every annual ivy pull that
has been organized. Not only has it been a great way to preserve the trademark trees that define our
neighborhood, but it is really the only organized neighborhood event that brings people together to get
to know each other better and form a better sense of community. I have gotten to know my neighbors
better through the ivy pull and the subsequent BBQ/gathering every year, and I know that it impacts
other neighbors in the same way.
In addition, each year we have held the ivy pull, the event has grown in popularity and participation. In
recent years, between 40 and 50 neighbors have turned out for the ivy pull. You can see pictures and
write-ups from past events on our web site, www.glenmorrie.wordpress.com. Our event this year will
take place on Sunday, May 5.
We would greatly appreciate another grant from the city to help us preserve the character of this
neighborhood and its natural beauty. And judging by the number of people who turn out every year to
help out, I am not alone in this sentiment.

the Glenmorrie Neighborhood has sent out in the past for notifying neighbors of the annual ivy
pull. In addition to the charges below ($41.24 for the cards, $13.76 for shipping, which was
waived on this sample order with a promo code, and $14.24 for labels) postage for these would
be $87.50, bringing the total to $142.98. If we went this route, we would also need to get the
list of addresses from the city and also find a way to print the mailing labels.

HHNA is requesting $300 to be used for a summer NA ice cream social. This will be
the fourth annual ice cream social. Each year this event has grown in popularity, with
over 80 neighbors attending last year's event. Funds will be used for food,
refreshments, etc. to help grow our event and serve more people.
The HHNA ice cream social request is for up to $300. Money will go towards
purchasing ice cream, disposable bowls and spoons, ice to keep the ice cream cold,
and other sundry products associated with the serving and eating of ice cream. Any
exceedance of $300 will be covered by private donations and will not be invoiced to the
NEP grant.
The ice cream social will be held at Freepons Park.

The ice cream social provides a public benefit by promoting a strong sense of
community within our neighborhood. All residents within the HHNA are invited.
Signs throughout the neighborhood will give public notice for the ice cream
social.

Members of the HHNA Board will work as volunteers to host the ice cream
social. Last year, several board members helped with set up, serving ice cream,
and clean-up at the end of the event.
Sarah Ellison is responsible for coordinating the ice cream social. Other HHNA
board members may also help.

Freepons Park is cleaned up, and all trash removed, at the end of the ice cream

annually, but each year the ice cream social is a discreet event, independent of
the previous year's ice cream social. It is not an ongoing event.

Hallinan Street and part of the Laurel Street Right of Ways cleared of invasive plants:
English ivy, English laurel, Himilayan Blackberries, etc. and planted with native species.

Clearing this property of invasive plants will preserve the large Douglas Fir trees
that are at risk (English ivy). They are a valued part of our neighborhood and
city, both aesthetically and environmentally. As we have worked in our
neighborhood natural areas (3 natural areas totaling 12 acres) to improve
wildlife habitat, this project would add to our efforts. Removing the invasive
species will also protect adjacent homeowner's from invasives intrusion into
their property. And, clearing this property will make it possible to eventually
build stairs between Laurel and Oak Streets that will improve our neighborhood
accessabiltiy and connectivity.
This project was discussed and unanimously supported by the Hallinan Heights
Neighborhood Association Board on April 4, 2019.

We would like to install a park bench, as pictured, behind the Little Library looking over
Freepons Park
The bench (pictured) is a Sterling Heights model which has been preselected by the
Parks Department. The cost is $1,500, according to Jeff Munro of the Parks Dept.
Installation will involve carrying the bench from a truck to the site, and is free, according
to Mr. Munro.
On a slightly raised patch immediately behind the little library in Freepons Park,
overlooking the park.

Proposal – New Park Bench in Freepons Park
The Hallinan Neighborhood Association thinks that Freepons Park would benefit from a new bench close
to the Little Library. We envision it as a peaceful place to read a book or to socialize with neighbors.
Freepons Park already has a couple of benches at its western end. This one would be to the east.
Although the eastern end of the park is very damp, almost swampy, during the rainy season, this specific
area is a bit higher and remains pretty dry.
The bench which we envision is pictured below. It is a Sterling Heights bench which has been pre-
selected by Jeff Munro of the Lake Oswego Parks Dept. The cost is $1,500. Mr. Munro says that the
Parks department can install such a bench for no additional (itemized) cost. Placing the bench on a
cement foundation, as pictured, will elevate it a bit further and keep it that much dryer.
Location: We propose to place the bench behind the Little Library looking over the park.

The Lake Grove Neighborhood Association (LGNA), at its April 18th meeting, voted its support for the
Neighborhood Enhancement Program (NEP) grant application for railings on the bridge over Springbrook
Creek. The proposed neighborhood enhancement will improve pedestrian safety.

SECTION 2: PROJECT DESCRIPTION, BUDGET & LOCATION IF APPLICABLE
Description:
Improve existing road divider located on Middlecrest Road Between Fairmont
Road and Kenwood Road
Replace existing retaining wall of railway ties which are decaying and mossy with
material that is durable and aesthetically pleasing, while keep existing footprint.
Improve upon the existing landscaping to create an visually pleasing space with
drought-tolerant and possibly native plants that would require minimal
maintenance and zero pesticides. Plants may encourage pollinators to our
neighborhood.
The replacement retaining wall and native/drought-tolerant plants selected for
this project will be coordinated with the city’s maintenance supervisor to ensure
they are not in conflict with city street operations and maintenance. Ideally would
like stone that is similar to the stone in Sundeleaf Plaza/Millennium Plaza. Plants
wise would keep existing maple trees, rearrange existing irises and tulip(so far I
only see one) to keep in the mix, add some rosemary, tall grass, to have plants
filling the space year round with some flowering ones as well. This project
should take 1 1/2 to 2 days and should only require a sign saying that it’s blocked
from pedestrian/bike traffic since there is no vehicular traffic at most.

Budget:
Funds would be used to pay removal and discarding of existing wall, replacing
with alternate material and labor involved, supplementing soil/mulch, adding
more plants to have ground cover year round.
Please note that unused funds are not transferrable to future grant cycles due to budgetary
reasons. If unexpected circumstances prevent projects from being completed, applicants
should resubmit their projects during the next grant application cycle. The Neighborhood
Enhancement Program will not fund projects from the City’s Capital Improvement Plan.

This divider is where many people walk and bike through this structure. It could

I am happy to work with the city and the landscaper that is selected to work
with. The possibilities are Mike Kebbe with Oswego Land Management,
Mitica Leontescu with Creative Fences & Decks, Randy Mihalko with
Storm Landscape Services and Sam DeFever with Dennis’ 7Dees.
• For physical improvements, be located on public land.
This is on public property, right on Middlecrest Road. I have been in
communication with Erica Rooney, the City Engineer as well as Pam
Peterson. Please see attached email correspondence.
5. The project is no or low-maintenance (and will be maintained by volunteers).
Other than the current arrangement the city has with a company that
checks on the plants on some regularity the goal is to have low
maintenance plants and low maintenance walls, initially the plants may
require some watering until the roots are established. Between myself and
other neighbors who are aware of the project we will tend to the watering
and care of the plants per recommendation of landscaper.
6. Does not fund ongoing or annual expenses.
This is a one time funding to complete the project.
7. For emergency preparedness projects, fund shared resources, rather than
individual preparedness items.

Hi Erica,
Eric referred me to you to run this potential Neighborhood Enhancement Project by you since it is on the
street.
Our Lakewood neighborhood is interested in replacing the wood retaining walls with something more durable
and yet attractive and possibly plant additional native plants in the beds, something that is drought resistant
since there is no irrigation for them. My understanding is this has been there for more than 25 years and it
showing signs of wear. It would be a nice place for people to sit for a moment. There's a lot of foot traffic/bike
traffic so it would be nice to improve it. It's a sort of zigzag formation I figure to avoid people from racing down

Yes, I am familiar with the installation. I believe we could entertain ideas for improvement, as
long as the general configuration and intent were maintained – allowing access for pedestrians,
without creating a “straight shot”. I like the idea of drought-tolerant plantings, etc. As long as the
ideas don’t become too complicated with things such as lighting, irrigation, etc. than we can take
a look at options for replacement or improvement.

Yes we would keep the same configuration, just looking to replace the wood for something else. Definitely not
looking for anything complicated-no lights or irrigation since there is no existing electrical or water sources
there.
Do you have resources on who would be able to do this? Any particular contractors/companies who have
done this sort of work for the city both retaining wall and planting? I am happy to reach out to them to get bids
when the time is right. Some of the nearby neighbors are happy to help with planting as well if needed. It
could be a community bonding project potentially.

We ask that all applicants to try and provide complete estimates, including labor, materials, and
any contingency amount that they believe might be necessary for their proposal. City staff can try
to fill in any gaps, but if there is not enough information in the proposal to evaluate, the committee
likely will decline it, or ask the NA to resubmit next year.
I understand you’re working with Pam Peterson on developing plans, and that you’re working with
the consultants to refine the cost estimates for your proposal. I just wanted to reiterate the
importance of having a complete estimate as a component of your application, so please try to
get those in by Friday in order to avoid any complications.

SECTION 2: PROJECT DESCRIPTION, BUDGET & LOCATION IF APPLICABLE
Description:
(Clearly describe your project in as much detail as possible. For neighborhood signs, please
include the proposed sign design, a site plan and photo(s) as attachments to the application.)
The McVey-South Shore neighborhood association is beginning to develop a neighborhood
emergency plan, and as part of that, we wish to purchase a 50 person medical trauma kit for
use by medical personnel in the event of a major catastrophe. It would be stored at the Lake
Oswego Corporation building at 700 McVey Avenue. This building is new within the year and
has been built to the highest seismic standard, referred to as Immediate Occupancy, which
should be safe to occupy after a Magnitude 9 earthquake such as the Cascadia Subduction Zone
quake. The fire department has access to the building and the Lake Corp management has
given us permission to set up a medical station there. We are forming a medical team that will
identify area doctors and nurses and plan how such a medical station will work. The first step in
that is stockpiling some basic supplies that can be accessed by whichever medical personnel are
the first to arrive. We envision this as a “second aid” station, since it is dependent on
availability of firefighters to open it up for us and do a quick inspection to make sure it’s safe to
occupy. We are preparing the residents to be each other’s own first responders, and to plan to
give first aid in their immediate areas right after the emergency happens. This would be a place
to come following that for checking in with a doctor, receiving follow-up care, or treatment for
more acute injuries and possibly transportation to the hospital.
Budget:
(Outline in as much detail as possible, how your requested funds would be used. Please note
that neighborhood parties, picnics, and other social events are generally limited to $300. If you
are purchasing more than $5,000 in goods or services from a single vendor/contractor, submit
copies of your bids with the application. See Program Guide p. 4, Step 7.)
The 50 person trauma kit, which has been pre-approved by the city, would be ordered on-line
and costs $239.97. Shipping is listed as 22.97, making the total cost $262.94. The

SECTION 3: CRITERIA
Please provide a narrative describing how your project meets the criteria listed below.
1. The project will provide a public benefit to the Lake Oswego community.
(Describe how your project will benefit the neighborhood and/or community. This is
your opportunity to demonstrate the benefit and value of your proposed project.)
Having a medical station at the Lake Corp building will benefit any neighbors within
walking distance who need medical care in a disaster. Those would include residents
from not only our neighborhood association, but also some from Hallinan Heights and
Lakewood neighborhoods. Having basic supplies already stockpiled would enable
providers to start treating injuries immediately. Additional supplies can be brought
there from neighbors’ homes as needs are determined. Meridian Park Hospital may be
hard to access because of road damage or blockage, and the hospital is expected to be
overwhelmed with serious injuries. We are trying to utilize local practitioners to treat as
many injuries as possible in order to reserve the hospital for injuries that can only be
treated there. There are many doctors and nurses who live in our neighborhoods, but
not everyone knows where they are and when they are home. Having a pre-designated
location where people can find help will be a huge benefit in promptly treating minor
injuries, in order to prevent them from becoming major injuries.
2. The project has neighborhood/community support and committed volunteers for
implementation.

support was demonstrated, i.e. number of votes at association meeting. In the space

the kit. We would make plans to replace those items at the appropriate time, at our
own expense, so this would be a one-time purchase.

Hi Mark,
No, the neighborhood association wouldn't want a key. We'd only go in when firefighters inspected and
opened it up. And possibly not until one of you guys arrived. The lake will slosh with a big earthquake,
endangering those on the shore. Mark mentioned that you may decide to draw the lake down a bit to
reduce risk from aftershock sloshing. We'd probably wait for you to make that call, depending on where
the water goes with the first shock.
The ad didn't specify the dimensions of the duffel, so I don't know exactly how big it is. It could be
stored anywhere it would fit and stay dry, definitely above floor level, as long as we would know where
to find it.

This is to confirm that this grant application was approved at our neighborhood
association general meeting on April 3, 2019. There were 19 people in attendance and
the vote was unanimous to apply for two 50 person trauma kits, to be stored at the Park
Academy and the Lake Corp building, pending final approval by each organization for
public use of their facilities in an emergency. I received word this week that the Park
Academy’s board decided that their primary responsibility was for the children in their
care and that they would be unable to open the school to the public, so we are not
applying for a trauma kit for storage there. At the same time, I received email
confirmation from the Lake Corp (attached) that they would be happy to make the
building available to the public and that they had room to store the kit, which comes in a
duffel bag. Hence the application for one trauma kit.

I don’t know if it’s a written requirement to have Prep LO support/review your EP grant but it
gives you a better chance at approval. You do need to schedule time with Erik to review your
app ahead of turning it in and the final due date. Hopefully, he can give you the OK to submit.

Thanks Rick, I gave this some thought overnight and see a slight difference between
this structure and David’s. These kits will not be stored at anyone’s home, but in
institutional buildings, which police or fire can open as soon as they inspect them for
safety. Therefore, the equipment would be accessible to whomever gets there first to
set up an aid station. That could be a CERT, a member of the Medical Team or an
organizer of the Emergency Plan. I see this kind of station as a place to provide
“second aid” since it depends on firefighters inspecting and opening. I would stress to
community members that they still need to be able to provide first aid to each other in
their immediate neighborhoods. This could be a place to come following that for
checking in with a doctor, having follow-up care, or treatment for more acute injuries
and possibly transportation to the hospital.
My understanding is that the kits come with an expiration date so we could build that
date into a schedule for replacement of specific items, which I note you can buy on line.
I’d expect the neighbors to bear that cost (about $50) just by taking up a collection. We
could designate a Responsible Managing Person to be in charge of the kits and their
updating as part of our Emergency Plan. The Medical Team has not yet been formed.
The Emergency Plan is just being organized.
I am awaiting letters from the school and Lake Corp granting permission for access and
confirming space for storage of the kits.
Does this sound like it would meet the criteria, specifically for access to the kits?

Just forward the application and letters.
Here’s some other requirements that need to be included in the app:
Who is the RMI (Responsible managing person) ultimately in charge of the kits?
David set the bar- you will need to have a total of 4 people living in your neighborhood to have
access to the kits. If the quake happens after hours, can any of the 4 people get the kits without
having to wait for someone outside your neighborhood to open the doors? These need to be
available 24/7 by the neighborhood.
Do you need a written agreement between PA, LC and the 4 people about access? If so, include
this with the app.
How often will they be inspected and updated?
What happens when one of your key people resign, move away, etc?
Sorry, the review committee at the city wants to make sure you always have the supplies
available for the whole neighborhood to use.
If you apply, why not ask for the $300 for meeting refreshments?

Introduction to Oswego Summit Condominiums, April 2019
The Safety and Preparedness Committee in our condominium community, Oswego Summit, is submitting a
grant proposal to the 2019 Lake Oswego Neighborhood Enhancement Program.
We are a 15-acre, 26-building, 214 owner-occupied condominium community located near the top of SW
McNary Parkway. Our community of approximately 350 residents includes younger families, with a majority
of older and retired persons.
We have had an active Safety and Preparedness Committee since 2006. Most recently we have expanded our
work to emphasize individual residents’ preparation for a minor or major ‘event,’ with The Big One as an
example.
To educate our residents we have designed and crafted posters expressing preparedness practices, written
supporting flyers, and have made them available at our community mailbox area. We assembled, in 2006,
what we call ‘The Red Book,’ and created a revised edition in 2017. We distribute The Red Book to every
resident and ensure new residents receive one when they move to Oswego Summit. This book includes the
Red Cross Prepare! and Living on Shaky Ground booklets and other sources for individual preparedness. At
our monthly board meetings, we give a progress report on our work and offer a safety and preparedness tip to
our residents. In the summer of 2018, our committee hosted a ‘preparedness’ table at our community
barbeque.
Despite our efforts of preparedness, we remain concerned with access to emergency services during an event,
especially since our CERT students were told our forested hillside location would make us isolated and difficult
to receive help.
Recently, our Board of Directors approved some funds to purchase equipment and medical supplies. This
effort is led by residents who have attended the Lake Oswego Community Emergency Response Team (CERT)
classes. In March, 2019, these same residents delivered three mini-classes for our community, passing on
highlights of what they learned in the CERT classes.
Along our way to greater preparedness we wanted to find an expert who would critique our ideas for
equipment and medical supplies before we solidified our list; our search led us to David Kainoa of the Lake
Forest neighborhood. David generously met with us twice, introducing our community to a water filtering
system we would not have found otherwise and communication radios which can be tuned to a nearby fire
station. He walked our property with the committee to advise where to put our cache of materials.
In reading the online Neighborhood Enhancement Program we see Lake Forest and Lakewood neighborhoods
applied for a grant in 2018. We believe our Oswego Summit Condominiums may be the only condo-complex
in Lake Oswego mirroring in some ways the preparations these communities are engaged in.
Our Board of Directors supports our committee’s submission of the Neighborhood Enhancement Program
grant request.

Section 3: Criteria

(CERT) graduates as well by neighbors trained to give immediate first aid care.

Addendum to Oswego Summit Condominium NEP Grant Proposal
This addendum addresses the suggestions made by the Prep LO review committee, in regard to our grant
proposal. We appreciate the opportunity to add this information.
• The Oswego Summit Responsible Managing Individual (RMI), in regard to the management of our
emergency supplies, is Cookie Johnson. In addition, three members of our Safety and Preparedness
Committee will also have access. These three members are Community Emergency Response Team
(CERT) graduates.
• We will develop an agreement to be signed by the four emergency key holders, ensuring when any of
us leave the community their key access to the storage area is surrendered. Another safety measure is
to re-key the lock when our group changes.
• The RMI and CERT residents will inspect the supplies in the spring of each year. Our plan has been to
use 40-gallon storage containers which have wheels and are easily moved using the attached handle.
• In our regular monthly meetings and via our community email list our residents will be notified of the
emergency supplies.

Oswego Summit is requesting the Heartsaver First Aid class described on this page. Heartsaver is a
four-hour class which can be delivered in our community lounge area. Each student receives a first aid

Heartsaver First Aid
This course is intended for people assigned as first responders to emergencies in the workplace or
community. The Heartsaver First Aid course provides training in basic first aid procedures, for those first
few critical minutes before emergency professionals arrive. This includes corporate employees, security
personnel, airline personnel, construction and manufacturing employees, and other individuals who want
or require first aid training.
Course content for First Aid:
General principles- Duties, roles, and responsibilities, First Aid basics and key steps, scene
safety, phoning for help, Universal Precautions and glove removal, finding the problem, and
optimal positioning for ill or injured persons.
Medical emergencies- Asthma, choking, allergies and epi pen use, chest pain and heart
attacks, fainting, diabetes and low blood sugar, stroke, and seizures.
Injury emergencies- External bleeding and control (bandage and tourniquets), penetrating and
puncture wounds, amputations, shock and internal bleeding, head and spinal injuries, injuries to
bones, joints, and muscles, and burns.
Environmental emergencies- Bites and stings, temperature-related emergencies, poisonings.
Heartsaver CPR/AED
Students practice right along with the video, which leads to longer-term retention of skills. Heartsaver CPR
is for employees who need CPR training and a course completion card.
Course Content for CPR:
- Scene safety and Assessment
- Push hard and fast: the kinesthetic feel of 100 – 120 compressions per minute
- Giving breaths: chest rise is always a good thing
- AED: shock therapy for better survival
- CPR: putting the two together to save a life

- Discussion of Naloxene use for medication (pain) overdoses
- Abdominal thrusts for the choking patient
The American Heart Association requires that each course participant have access to a copy of the training
textbook, Heartsaver First Aid with CPR/AED. Course completion results in an American Heart
Association certification card which is valid for two years.

Our neighborhood as well as the community will benefit from the removal of weeds, the
addition of plants and mulch on the S. side of the main entry to East Waluga Park as well
as along the fence adjacent to the pathway. Our project efforts will greatly enhance the
appearance to this very visible area of East Waluga Park, since there exists a heavily used

I have been working with Cheryl and her group on a planting on the south side of the entrance of
East Waluga park. Last year we planted the north side and would like to match up the other. The
project would include prepping the soil, purchasing the plants(I will help with), installation and
mulching the area. If there are enough funds the plantings will be extend along the pathway.

SECTION 2: PROJECT DESCRIPTION, BUDGET & LOCATION IF APPLICABLE
Description:
For the neighborhoods surrounding Southwood Park, the park represents a treasured gem
perfect for walks and family activities. As the only home owner’s association contiguous to the park,
Westwood has partnered with the city supported Friends of Southwood Park group and the City’s
Parks & Recreation Department for over 20 years. During that time, Westwood neighbors and many
others, have made improvements there to reduce invasive plant propagation, clean-up trash, and
improve pathways several times each year. The Westwood HOA board of directors has voted to seek
the addition of two park benches to aid walkers, and to provide an additional incentive for people to
spend a few more minutes in the park. Currently, there are no benches or picnic tables in the park.
To reduce grant costs, the Westwood HOA board proposes to provide all the labor necessary
to install the benches in the locations specified by the Parks & Recreation Department, and with their
oversight of the work quality.
In preparing a first draft of this NEP grant proposal, the Westwood HOA researched and
suggested several bench designs that would make Southwood Park unique from other city managed
parks. However, after discussions with Park’s supervisor, Jeff Munroe, we agreed to use the bench
design chosen by the LO Parks team. The detailed costs breakdown below reflects that bench model
and the cost estimate provided by Mr. Munroe. Here is a small photo of bench model below.

Budget:
As just mentioned, labor is not included in this grant proposal’s budget, since it will be
performed by nearby neighbors from the Westwood HOA. Likewise, all construction tools needed
will be borrowed from these same nearby neighbors. However, LO Parks & Recreation department
personnel may be temporarily needed to allow trucks with construction materials to drive into the
park to place materials next to the construction sites.

a red arrow in the aerial photo below; however, the LO Parks team will determine the best
placement for these benches based on future plans for the park.
SECTION 3: CRITERIA
Please provide a narrative describing how your project meets the criteria listed below.
1. The project will provide a public benefit to the Lake Oswego community.
Currently, there are no park benches or picnic tables in Southwood Park. Consequently,
people tend to simply walk through the park without stopping to enjoy to newly planted
native plants, stream, or fauna. Even just giving a person a bench while their dog plays, or a
place to chat with another park visitor means that those people will spend just a little more
time appreciating our significant asset.
2. The project has neighborhood/community support and committed volunteers for
implementation.
The Westwood board of directors (listed) below voted unanimously in favor of adding 2
benches to the park, and for applying for an NEP grant to help fund it. They have also
committed to the volunteer hours mentioned above to get the benches installed.

Likewise, the president of the Oak Creek Neighborhood Association (Teresa Spangler) has
voiced her support for the project, and provided a written endorsement on behalf of the Oak
Creek Neighborhood Association (please see the attached).
Finally, the Friends of Southwood Park group has given this project their support also.
3. Responsibility:
The president of the Westwood HOA, Darrel Young, has committed to be the coordinator and
contact point between the LO Parks & Rec Department, the volunteers, and the material
supply vendors. As such, he will target a weekend for the bench installation, insure that
enough volunteers and borrowed tools are present to complete the installation, order all
materials and have them delivered shortly in advance of the project weekend. He will
coordinate with LO Parks & Rec personnel who can oversee the project before, during, and
after the project weekend.
4. For physical improvements, be located on public land.
The aerial photo on the previous page depicts Southwood park, and indicates the proposed
positioning for each bench. The initially proposed positioning is well within the park
boundaries. The Westwood HOA president will follow the direction of the LO Parks & Rec
team as to the best locations for the benches.
5. The project is no or low-maintenance (and will be maintained by volunteers).
The benches proposed are no maintenance structures. In addition, to making grass mowing
easier the new cement slab to which the benches will be bolted will be bordered by mulch
with landscaping cloth underneath to inhibit weed growth. The metal frames of the benches
are power-coated enamel, and so should not require repainting.
6. Does not fund ongoing or annual expenses.
No expenses are required beyond the initial installation of the benches.
7. For emergency preparedness projects, fund shared resources, rather than individual
preparedness items.

8. Neighborhood parties, picnics and other social events are limited to $300 in grant funds.

9. The project can be completed and expenses incurred by June 30, 2020, the end of the
grant cycle. All invoices and/or requests for reimbursement submitted to the City no

the April 26th filing deadline. This is the first time that the Westwood HOA has applied for one of
these city grants, so we’ve been reading the filing instructions carefully. Since the Westwood HOA is
part of the Oak Creek Neighborhood, and as the grant application instructions mention, I plan to seek
an endorsement from Teresa Spangler (Oak Creek NA president) also. Do you know of anyone else
that I should contact in advance of submitting the application?
I have attached our first draft NEP application if you’d like to read the details, see the photos, and
review the proposed costs in detail. The goal is to place 2 park benches in Southwood Park. As you
know, there aren't any there now. There is a photo with the proposed bench locations indicated on
an aerial view of the park, but it would basically be one bench in each of the two “zones” within the
park. The NEP grant funds would purchase the benches themselves, and the various materials
required to pour a supporting concrete slab, with a small gravel perimeter to ease mowing. We
propose that the Parks and Recreation Department choose one of the three bench options, or
suggest a vendor and park bench model that we have not considered. Volunteers from the
Westwood HOA, coordinated by me, would make all the purchases, arrange material delivery timing,
supply all the hand-tools, and actually perform the installation. All this, of course, would be
supervised by your team to whatever extent you'd like.
So, my request of you would be, first, to let me know what you think of the general idea. Mary
Solares and the Friends of Southwood Park are on-board for this, as is the whole Westwood HOA
board. Second and third, would be choosing one of the three bench model options, and generally
blessing the locations for the 2 benches. Having not been through this process before, I would
greatly appreciate your advice and direction on how best to accomplish our goal.

grant application. She tells me that I should probably take the application first to the Parks Maintenance Superintendent,
Jeff Munro (jmunro@lakeoswego.city). Do you think that is the best approach? I assume you work with Jeff. Have you
by chance discussed our NEP grant application with him? I want to get my ducks in a row this week, since the filing
deadline is next week.

Parks and Recreation Department choose one of the three bench options, or suggest a vendor and park
bench model that we have not considered. Volunteers from the Westwood HOA, coordinated by me,
would make all the purchases, arrange material delivery timing, supply all the hand-tools, and actually
perform the installation. All this, of course, would be supervised by your team to whatever extent you'd
like.
So, my request of you would be, first, to let me know what you think of the general idea. Mary
Solares and the Friends of Southwood Park are on-board for this, as is the whole Westwood HOA board.
Second and third, would be choosing one of the three bench model options, and generally blessing the
locations for the 2 benches. Having not been through this process before, I would greatly appreciate
your advice and direction on how best to accomplish our goal.
