When the family needs assistance, our Client Services Coordinator can arrange to meet with family members about issues such as health & well being, safety, care giving, housing, long-term care, and respite services. 
 
Home Visits
The Client Services Coordinator is available to visit the home and help coordinate services and assist families in resolving problems and maximizing independence.
