Dog License Application Instructions

Step 1:
Fill out the dog license application form by either:

• Create an online account by going to the Licensing ePortal and log in to apply or renew

• Complete the attached application form in Acrobat Reader. (You may want to save a copy for future reference); or
   
• Print the attached application and fill in by hand

Step 2:
Payment options:

• Pay online through the Licensing ePortal  (after the application is approved); or
• Mail in the application and required documents with a check (see mailing address below); or
• Pay by check in person at the Finance department on the third floor of City Hall.
