Dog License Application Instructions

Step 1:
Fill out the dog license application form by either:

  • Create an online CAP account by going to the Government Connected Online Services webpage here and either just click on Create a New Account or click on the CAP instructions (pdf) in the middle of that page. (Only need to create the CAP account once, then can use for renewals.) 

    • Then complete an online dog license application (click the Apply or Renew Dog License pdf in the middle of that same webpage above to follow instructions); or
  • Complete the attached application form in Acrobat Reader. (You may want to save a copy for future reference); or
  • Print the attached application and fill in by hand

Step 2:
Payment options:

  • Pay online in your CAP account (after the application is approved); or
  • Mail in the application with a check or fill in the credit card information (see mailing address below); or
  • Pay in person at Finance department on the first floor of City Hall during regular business hours (8am - 5pm).  Must bring rabies and sterilization certificate.  (Exception when renewing dog license — do not need the rabies certificate if it has not expired or will not expire for the period that you are getting the license.)
