Passport Service at City Hall
The City of Lake Oswego Finance Department is an official Passport Acceptance Facility that accepts applications for passports and passport cards and forwards them to the Regional Office of the United States Passport Agency.

Passport Information
A passport is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid United States (US) passport book, passport card, or WHTI compliant document is required to enter and leave most foreign countries.
Many countries around the world have instituted a six-month rule, which requires that travelers have at least six months of validity left in their passports at the time of arrival. For more information go to the US Department of State website ( https://travel.state.gov/content/travel/en/international-travel.html ) and search by country.
Passport applicants must apply in person with required documents. Minors and infants must also be present. (See exceptions for those eligible to renew by mail.)
Application forms for Passport Books and/or Passport Cards are available at the Finance Department, on the third floor in City Hall, in the forms holder on the wall by the finance counter. Or you may download an application at the US Department of State website (printed one-sided)

Renewals
To see if you are eligible to renew your passport directly by mail go to this US Department of State website.
(If you plan to expedite the application, be sure to write "EXPEDITE" clearly on the outside envelope.)

Please be sure to have the following documents when applying for a passport book or passport card:

Two identical passport photos (2 inches by 2 inches) taken within the last six (6) months. Glasses are not acceptable in passport photos.

Proof of citizenship. Birth certificate, a previously issued US passport, or a certificate of naturalization. For Minors a birth certificate showing the names and relationship* of both parents must be submitted for each child age 15 and younger.

Current picture identification, such as a state driver's license or ID card.

Check or money order for payment to the US Department of State

Expedite Fee* (Optional $60) - If you are planning on traveling within 8 weeks of submitting your application it is recommended that you pay an expedited processing service fee to the US Department of State for an additional $60.

Return Express Mail fee** $21.36 for 1 - 2 day delivery service (payable to the US Department of State)
* Passport Book, Passport Card, Expedite Fee, and Return Express Mail fees are made payable to the US Department of State… only by check or money order.
** These fees are payable to the City of Lake Oswego are by cash, check or credit card. (Execution/processing fee is per application. Passport books and Passport cards can be applied for together on one DS-11 application.)
