Volunteer Forms
To become a volunteer at the LOACC, you must please complete the following forms to be considered. Please note that since we serve a vulnerable population a background check must be completed and approved by the City of Lake Oswego and the Oregon Criminal History and Abuse Records Data System (ORCHARDS). Due to the volume of volunteers, the background check through the State of Oregon may take 6-8 weeks to be cleared and approved.
Fingerprinting in the Background Check Process
Subject individuals may be required to have a fingerprint-based check as part of the Background Check Unit (BCU's) criminal records check.
